✅ Eligibility for PMJAY G Card

🧾 Process for Serving Government Employees:
1. Certificate Collection
- Fill up the Certificate Format decided by your department.
- Get it signed & sealed by Head of Department.
2. Submit Certificate
- Submit the certificate to your DDO (Drawing & Disbursing Officer) or PMJAY Nodal Officer.
- They will forward the details to SHA (State Health Agency), Gujarat.
3. E-KYC Verification
- Do Aadhaar-based e-KYC for you & dependents.
- Can be done Online or through Camp.
4. G Card Generation

👴 Process for Pensioners:
- Get Certificate from District Treasury / Sub-Treasury / Pension Payment Office.
- Certificate will be forwarded to SHA Gujarat.
- Do Aadhaar-based e-KYC for self & dependents.
- After verification, your PMJAY G Card will be generated.
